MIC \u2265 2 \u00b5g\/ml \u0111\u1ed1i v\u1edbi ceftriaxone \u0111\u01b0\u1ee3c xem l\u00e0 ng\u01b0\u1ee1ng \u0111\u1ec3 x\u00e1c \u0111\u1ecbnh vi khu\u1ea9n s\u1ea3n sinh ESBL, tuy nhi\u00ean c\u00e1c x\u00e1c \u0111\u1ecbnh n\u00e0y c\u00f3 h\u1ea1n ch\u1ebf \u0111\u1ed1 v\u1edbi nh\u1eefng vi khu\u1ea9n kh\u00f4ng nh\u1ea1y v\u1edbi ceftriaxone v\u00ec l\u00fd do kh\u00e1c ngo\u00e0i s\u1ea3n sinh ESBL. Trong h\u01b0\u1edbng d\u1eabn n\u00e0y ESBSL-E \u0111\u01b0\u1ee3c xem l\u00e0&nbsp;<em>E. coli, K. pneumoniae, K. oxytoca,&nbsp;<\/em>ho\u1eb7c&nbsp;<em>P. mirabilis.<\/em><\/p>\n\n\n\n<p><strong>C\u00e2u h\u1ecfi 1: Kh\u00e1ng sinh n\u00e0o n\u00ean \u0111\u01b0\u1ee3c l\u1ef1a ch\u1ecdn \u0111\u1ec3 \u0111i\u1ec1u tr\u1ecb vi\u00eam b\u00e0ng quang kh\u00f4ng ph\u1ee9c t\u1ea1p g\u00e2y ra b\u1edfi ESBL-E?<\/strong><\/p>\n\n\n\n<p>Nitrofurantoin v\u00e0 TMP-SMX l\u00e0 nh\u1eefng kh\u00e1ng sinh \u0111\u01b0\u1ee3c \u01b0u ti\u00ean l\u1ef1a ch\u1ecdn cho b\u1ec7nh nh\u00e2n vi\u00eam b\u00e0ng quang kh\u00f4ng ph\u1ee9c t\u1ea1p do nhi\u1ec5m ESBL-E. Ciprofloxacin, levofloxacin v\u00e0 carbapenem l\u00e0 nh\u1eefng t\u00e1c nh\u00e2n thay th\u1ebf cho b\u1ec7nh nh\u00e2n vi\u00eam b\u00e0ng quang kh\u00f4ng ph\u1ee9c t\u1ea1p do ESBL-E. M\u1eb7c d\u00f9 hi\u1ec7u qu\u1ea3, tuy nhi\u00ean ciprofloxacin v\u00e0 levofloxacin kh\u00f4ng \u0111\u01b0\u1ee3c khuy\u1ebfn ngh\u1ecb n\u1ebfu nitrofuratoin v\u00e0 TMP-SMX v\u1eabn c\u00f2n hi\u1ec7u qu\u1ea3. Ngo\u00e0i ra, aminoglycosides li\u1ec1u \u0111\u01a1n v\u00e0 fosfomycin \u0111\u01b0\u1eddng u\u1ed1ng (ch\u1ec9 \u0111\u1ed1i v\u1edbi&nbsp;<em>E. coli<\/em>) c\u0169ng l\u00e0 nh\u1eefng li\u1ec7u ph\u00e1p thay th\u1ebf cho vi\u00eam b\u00e0ng quang kh\u00f4ng ph\u1ee9c t\u1ea1p do ESBL-E.<\/p>\n\n\n\n<p><strong>C\u00e2u h\u1ecfi 2. Kh\u00e1ng sinh n\u00e0o \u0111\u01b0\u1ee3c \u01b0u ti\u00ean l\u1ef1a ch\u1ecdn trong \u0111i\u1ec1u tr\u1ecb vi\u00eam b\u1ec3 th\u1eadn v\u00e0 nhi\u1ec5m tr\u00f9ng \u0111\u01b0\u1eddng ti\u1ebft ni\u1ec7u ph\u1ee9c t\u1ea1p do ESBL-E?<\/strong><\/p>\n\n\n\n<p>TMP-SMX, ciprofloxacin ho\u1eb7c levofloxacin l\u00e0 nh\u1eefng kh\u00e1ng sinh \u0111\u01b0\u1ee3c \u01b0u ti\u00ean l\u1ef1a ch\u1ecdn trong \u0111i\u1ec1u tr\u1ecb vi\u00eam b\u1ec3 th\u1eadn v\u00e0 nhi\u1ec5m tr\u00f9ng \u0111\u01b0\u1eddng ti\u1ebft ni\u1ec7u ph\u1ee9c t\u1ea1p g\u00e2y ra b\u1edfi ESBL-E. Ertapenem, meropenem v\u00e0 imipenem-cilastatin l\u00e0 nh\u1eefng kh\u00e1ng sinh ch\u1ec9 n\u00ean \u0111\u01b0\u1ee3c l\u1ef1a ch\u1ecdn trong tr\u01b0\u1eddng h\u1ee3p \u0111\u1ec1 kh\u00e1ng ho\u1eb7c c\u00f3 \u0111\u1ed9c t\u00ednh v\u1edbi TMP-SMX ho\u1eb7c kh\u00e1ng sinh nh\u00f3m fluoroquinolone. Aminoglycoside (v\u1edbi li\u1ec7u tr\u00ecnh \u0111i\u1ec1u tr\u1ecb \u0111\u1ea7y \u0111\u1ee7) c\u0169ng l\u00e0 m\u1ed9t l\u1ef1a ch\u1ecdn thay th\u1ebf trong \u0111i\u1ec1u tr\u1ecb vi\u00eam b\u1ec3 th\u1eadn v\u00e0 nhi\u1ec5m tr\u00f9ng \u0111\u01b0\u1eddng ti\u1ebft ni\u1ec7u ph\u1ee9c t\u1ea1p do ESBL-E.<\/p>\n\n\n\n<p><strong>C\u00e2u h\u1ecfi 3.
